Purpose: To consolidate the multiple public health departments of Tippecanoe County into a unified Tippecanoe County Health Department ("HD"). An integral aspect of the consolidation is the establishment of a plan whereby those individuals who are presently working for the City of Lafayette, Board of Health, become employees of Tippecanoe County. Organization: Joint agreement. No governing board. Joint supply: City contributes equipment and County provides compensation for former City employees who decide to become County employees. (Pp. 2, 3)

Staffing and supplying: In order to establish and efficiently maintain HD, individuals presently employed by City as employees for Board of Health shall become employees of Tippecanoe County. Such employees will carry over any accrued benefits as of January 1, 1994, and any benefits where years of service is a factor will also be carried into new positions. (P. 2) Equipment used and owned by City Board of Health shall become the property of the HD. List of equipment to be transferred to County is included, with description of each item, how many of each item, and appraised and replacement values for each item. Categories of equipment are: Office Equipment, Mosquito Equipment, Nursing Division, Miscellaneous Office Items. (P. 2-3)

Financing: County will fund and provide compensation to certain employees (a list of 8 names is included) provided they desire to become County employees. County will fund and provide the following benefits for the above mentioned employees:

a) PERF (a retirement benefit account);

b) Sick leave: City employees may transfer, and County will assume liability for, up to 36 days of sick leave;

c) Vacation: Same as for sick leave, but only 5 days. (P. 3-4)

Administration: The Indiana State Legislature has mandated that any Indiana counties which have multiple health departments merge them into one County Health Department as of January 1, 1994. (P. 1) City and County both agree to cooperate fully in carrying out this agreement, which may include: adopting resolutions, appearing at all public or private meetings, providing statistical and technical data, and executing documents or writings. Termination: Shall not occur except by mutual written agreement of the parties. (P. 5)
