Purpose: To provide rabies-testing services to the residents of Allen County who live outside the City of Fort Wayne. (P.1)

Organization: Joint agreement. No joint board. Single supply: City will provide the personnel and facilities needed (through the Fort Wayne Department of Animal Control). (P. 1)

Duration: Indefinite. (P. 2)

Services delivered: Bite investigation and enforcement will be performed by the Department for incidents occurring inside the City limits only. Euthanasia, processing, and submission of animal heads will be provided by the Department for all Allen County bite cases as needed. (P. 1)

Financing: Board of Health will pay City $23,000 for services delivered during 1993. This sum is due and payable each year this agreement is in effect. Payment is made in two equal installments: First, on or before May 1, second, on or before December 31. (Pp. 1-2)

Administration: Fees and policies for owned quarantined animals are identical for City and non-City residents. (P. 1) Quarantine and euthanasia decisions and enforcement in high risk situations will be made by the Allen County Board of Health in consultation with the Department. (P. 1)

This contract is effective so long as the City has contractual agreements for use of their animal shelter with the Allen County Sheriff's Department and the Allen County Board of Health. The terms of this agreement run concurrent with these other two. (P. 2)

Termination: December 31, 1993. But renewal of the contract is automatic unless one of the parties withdraws. Notification of withdrawal made in writing prior to May 1. City reserves right to terminate contract if payment is at any time not forthcoming or insufficient. (P. 2)
